1:5 to 1:8 by weight for concentrate you will dilute. 1:8 (100 g : 800 g) is a common starting point; dilute 1:1 with water or milk over ice.
No — at 1:5–1:8 it is a concentrate with far more extracted solids than hot coffee. Dilute 1:1 to 1:3, or 1:2–1:4 with milk.
Cold water extracts more slowly and less fully, so you compensate with more coffee per litre and a long (12–18 h) steep.
